Please fill out the registration form below. Note that upon completion, you will receive a confirmation email. In about 5-10 business days you will receive temporary donation envelopes so you may begin offering to St. Monica's. In about 4-6 weeks, you will receive your personalized donation envelopes with your official church envelope number.
